Welcome to the HealthBASE Registration process. The first part of this process is to complete the subscriber agreement between your organization and HealthBASE. You will need to provide information about your organization and your administrative user to HealthBASE. You should be a representative of your company who is able to enter into a contractual arrangement with HealthBASE. After HealthBASE verifies the information that you have submitted, your administrative logon will be enabled. Once enabled, you will be able to complete your online profile, create additional users and manage your account.
